Engooden Health transformed the care delivery model for chronic disease management by providing consistent, trust-based patient engagement between physician visits.
Engooden offered payers and providers scalable, technology-enabled services for patients living with chronic conditions. Their approach focused on rising-risk patients: a significant population that is often difficult to reach. Engooden worked to alter the trajectory of those patients’ health outcomes, with a goal of improving their overall quality of life and keeping them out of the high-risk population. The name reflected that mission: to transform something negative into a positive; to ameliorate or improve. That was what the team set out to do every day for its customers and the patients under their care, whether by helping patients manage diabetes or high blood pressure, navigate medication and prescriptions, or provide emotional support through a monthly touchpoint. Their goal was to help patients improve their health and quality of life.
